REQUIREMENTS FOR PARTCIPATION IN ATHLETICS
AT MIDDLETOWN HIGH SCHOOL & GAUDET MIDDLE SCHOOL:
 
Students interested in participating in any sport offered by the Rhode Island Interscholastic League must complete the following:
 
  • Registration on rSchool (for EVERY season and EVERY sport). Click on the icon above and following the directions.  

     

  • Completion and submission of the RIIL Assumption of Risk Form. Sign and have it notarized before turning in to the appropriate coach. NO student will be able to participate in tryouts without this form turned in. The RIIL requies each student to complete this form one time in MS and again one time in HS. If you child has already done this for a Fall sport or in a previous school year, IT IS NOT REQUIRED AGAIN. You do need to fill out another one if you had previously done so in a different school district. 
  • Uploading of Current Physical: A current Physical (valid for 12 months, from date indicated on the form) should be uploaded in the athlete's rschool.com account prior to participation in any tryout or practice, and no later than two days prior to the first day of practice/tryout. Also, the physical should be dated and signed by an authorized physician.
